if you do not have a USA Boxing passbook, you must have all necessary documents uploaded in your usa boxing account (face photo, birth certificate, current copy of transcripts or classes as long as it shows name, school and hours, and current physical cleared to compete), and email request at least 3 weeks prior to the event!
